Request Funds

Please complete and submit this request form for funds approval 2 weeks in advance, or earlier, of when funds are needed for PTO review. 

*Requestor must be an ACTIVE member with the PTO at Pleasantdale Elementary School. Active is defined as assisting/volunteering with PTO events.
